Comprehensive Behavioral Health Services
During Intake, our clients meet with a trained Mental Health Professional (MHP). During this session, the client is thoroughly explained, reviews, and signs their Intake documents (e.g., Grievance forms, Client Rights & Responsibilities, Consents, etc.).
Mental Health Case Management includes the following: assessment, planning, coordination and advocacy services for clients who need multiple services and require assistance in gaining access to and in using behavioral health, physical health, social, vocational, educational, housing, public income entitlements and other community services to assist the client in the community. Mental Health Case Management may also include identifying and investigating available resources, explaining options to the client, and linking the client with necessary resources.
Assessments are deemed to be medically necessary and is provided to our clients after they complete Intake. Assessments and treatment planning is provided by qualified clinicians. Further, treatment planning can take place on-site or off-site (e.g., office, telehealth, community). Hence, the purpose of these tools is to assess and/or review the need to initiate or continue therapeutic services, for the purposes of diagnosis development or confirmation, and to develop, review, or update the clients’ treatment plan goals, objectives, and/or recommended treatment services.
Therapy/Counseling services are deemed to be medically necessary. Therapy/Counseling targets the immediate amelioration of a client’s presenting clinical issues. Additionally, Therapy/Counseling services may be provided prior to the completion of the client’s IATP. Therapy/Counseling includes all treatment modalities and clinical techniques, with an emphasis on evidence-informed practices, used by the therapist/counselor to promote positive and/or pro-social emotional, cognitive, behavioral, or psychological changes with the client. Depending on the presenting issues and program, clients will engage in therapeutic sessions 1—2 times per week. During sessions, clients will engage and work on treatment plan goals while building capacity.
Support Groups recognizes the importance of client interactions with fellow peers in addition to developing strong interpersonal, and self-awareness skills. These educational, therapeutic, and support groups provides in-depth life skill planning and training, along with group education, group coaching, and motivational coaching. Support groups give clients a chance to observe they are not alone. PDCS curriculum will teach client’s the following skills:
Crisis Intervention services performed by our qualified staff for the purposes of treating or ameliorating decompensation, loss of role functioning, or inability to deal with immediate stressors, resulting in a behavioral health crisis are deemed to be medically necessary.
Client-Centered Consultation Case Management includes client-specific professional communications among PDCS staff or between PDCS staff and staff of other agencies and community partners who are involved with service provision to the client. Professional communications include offering and/or obtaining a professional opinion regarding the client's current functioning level or improving the client’s functioning level, discussing the client’s progress in treatment, adjusting the client’s current treatment, or addressing the client's need for additional or alternative mental health services.
Community Support includes the facilitation of illness self-management techniques, identification and use of natural supports, development of functional, interpersonal and community-based coping skills, and other clinically informed efforts to support the recovery of the client. Additionally, Community Support includes efforts to increase targeted strengths or reduce targeted needs, as identified in the client’s IATP. Our community individual support services are not only centered on treatment services for the client’s mental health but also centered on maintaining relationships with area employers who welcome job seekers who are prepared and supported by our agency.
Medication Training is provided by a qualified staff who will engage and train clients on self-administration and safe-guarding (e.g., adverse reactions, storage, etc.) of medication and communication with other professionals, family and/or caregivers on medication usage, potential issues, and means to actively seek assistance in the event of an issue or emergency.
The MHE is a method our qualified staff uses to document a client’s basic cognitive, behavioral, and emotional functioning at a given point in time. The MHE includes a structured interview and systematic behavioral observations. Domains covered in the MHE include:
PDCS Violence Prevention Program team works alongside community partners. Our qualified team will deliver Violence Prevention services throughout Chicagoland (Cook County) and surrounding communities. Our team works with youth to 65-year-old adults. We utilize a client-centered and age-appropriate approach. We focus on helping individuals develop the knowledge, skills, and attitudes they need to make good choices and change harmful and risky behaviors. The curriculum includes but is not limited to; Gang-related issues, mentoring, peer-pressure, leadership, and the dangers of using illicit substances.

We provide comprehensive Job Readiness services to help veterans successfully enter or re-enter the workforce. Our programs include career counseling, resume building, interview preparation, and job search assistance tailored specifically to the unique skills and experiences of veterans. Additionally, we collaborate with city and state employers and service programs to provide a range of opportunities, which in fact helps facilitate the labor force, and enhances the success factor in long-term recovery. We further assist all academic, and skill levels to participate in skill-based training and secure employment, support services, apprenticeships, and jobs.
Our Referral Services connect veterans with the resources they need, whether it's for healthcare, housing, education, or specialized mental health support. We work closely with a network of trusted partners, including local organizations, government agencies, and non-profits, to ensure veterans receive the comprehensive care and assistance they deserve. Whether it’s accessing VA benefits, finding affordable housing, or connecting with specialized medical providers, our referral program is here to guide veterans to the right resources.
Our qualified team will engage, assist and educate families with obtaining a General Educational Diploma (GED); providing College Preparatory Workshops (e.g., Financial Aid, Application, etc.); College & Trade School assistance; Professional Guest Speakers, and Tutoring.
